These programs serve a variety of population groups including Medicaid recipients, elderly, Veterans with VA benefits, and people with intellectual or developmental disabilities, physical disabilities or other illnesses.

Responsibilities:
Manage care for clients who receive in-home personal care services Participate in Comprehensive Plan of Care (CPOC) planning meetings Create and implement client-specific Care Plans based on the State CPOC and with input from the client or their representative, ensuring that clients' medical, behavioral, and mental health needs are met Supervise, train, direct, and evaluate Caregivers in regards to all aspects of client care, ensuring Care Plans are followed and that clients' health and well-being are maintained Communicate frequently with clients, families, and other affiliates to answer questions, resolve issues, and ensure satisfaction with services Travel to clients' homes for meetings and supervisory visits quarterly, annually, and as needed to remain updated on client/family's satisfaction with services, health and well-being, living conditions, and progress toward goals Conduct regular Supervisory Monthly Report / Quality Assurance phone calls with all clients, and other phone contacts as needed Respond to critical incidents & complete Critical Incident Reports (CIR) within mandated timeframe Properly document all client communications, care plan or medical changes, visit notes, or incidents.
Maintain updated and accurate digital records for each client Communicate with Director, Staffing Coordinators, and HR on a regular basis to discuss any needs or issues regarding clients or employees Communicate with Finance Coordinator regarding Money Management services for specific clients Follow directives related to Emergency Preparedness & Response in dealing with natural or manmade disasters, assisting with client contacts and support as needed Promote LHCL in the local community within allowed standards and establish positive and professional relationships with external stakeholders Participate in after hours on-call rotation and serve as backup staff working in-home with clients as needed Follow policies, procedures, and guidelines established by LHCL, Medicaid program manuals, and LDH-HCBS licensing standards Model the LHCL Mission, Values, & Leadership Behaviors Additional tasks related to Client Care as needed Requirements:
Bachelor's degree required, preferably in Human Services or related field 2 years' experience preferred (HCBS industry, case management, or other related experience) Compassion and eagerness to serve elderly and disabled individuals Strong management, communication and organization skills Basic computer skills including Windows & Microsoft Office Able to operate office equipment Basic addition/subtraction and general analytical skills The ability to interact on a professional level with LHCL clients and their families, DSWs and representatives of other community-based organizations.
Valid driver's license, auto insurance, vehicle inspection, and reliable personal transportation; willingness to transport clients in personal vehicle Must satisfactorily pass all required and applicable State & Federal eligibility verifications including background, driver's license, and other required screeningsMust be able to read, write, and follow instructions in English Physical requirements include stooping, kneeling, reaching, standing, walking, and climbing stairs.
Must be able to occasionally lift up to 50lbs (carrying files/boxes, assisting with client lifts/transfers, etc.
